• Review the list of parks and destinations provided.
• Read the Terms & Conditions on the reverse side of the Vacation Request form thoroughly to ensure a complete understanding of how the reservation process works.
• Select the theme park, two accommodation choices for the park and the preferred dates of travel.
• Complete the Vacation Request form and mail or fax it directly to Motivation Advantage, Inc.
