• Read the Terms & Conditions on the reverse side of the Cruise Vacation Request form thoroughly to ensure a complete understanding of how the reservation process works.
• Complete the Cruise Vacation Request Form by indicating two(2) choices and mail or fax it directly to Motivation Advantage, Inc.
• Once your cruise request is received, Motivation Advantage, Inc. will contact you to review and finalize the booking of your cruise.
• We will confirm the reservation and send all of the cruise documentation to you.
